Denver Colorado Post Acute Care Physician Assistant Position-Flexible Scheduling

Physician Assistant: Physician Assistant | Colorado | Job ID: JO-2609-103573

A growing, physician-led medical group dedicated to providing high-quality care in Skilled Nursing, Long-Term Care, Assisted Living, Independent Living, and Rehabilitation settings is seeking a compassionate, motivated Physician Assistant (PA-C) to join its Denver-area team.

Responsibilities

  • Provide medical care and follow-up for patients in post-acute care settings.
  • Collaborate with physicians, therapists, case managers, and facility staff to coordinate patient care.
  • Complete timely and accurate clinical documentation.
  • Participate in quality improvement initiatives and ensure compliance with regulatory requirements.
  • Travel to assigned Skilled Nursing, Assisted Living, and Independent Living facilities throughout the Denver metro area.

Qualifications

  • Graduate of an accredited Physician Assistant program.
  • NCCPA certification required.
  • Active Colorado PA license, or ability to obtain.
  • Post-acute care, geriatrics, family medicine, internal medicine, or skilled nursing experience preferred but will consider new graduate PA's.
