A respected regional healthcare system in central New Hampshire is seeking an experienced Primary Care Nurse Practitioner to join a collaborative and growing outpatient practice. Position Overview
The Nurse Practitioner will provide comprehensive adult primary care services to patients aged 16 and older, with the majority being 18+ years of age. You will join an established team dedicated to delivering high-quality, patient-centered care in a supportive and collaborative environment.
Practice Highlights
- Outpatient primary care setting
- Patient population ages 16+, primarily adults
- Average of 16 patients per day
- Schedule consists of:
- Four 8-hour clinic days per week (8:00 AM – 5:00 PM)
- 8 hours of administrative time weekly, which may be completed remotely
- Established team currently includes 3 primary care providers, 3 nurses, and 1 medical assistant with the plan to expand to a total of 5 providers
- Two behavioral health providers see patients in the clinic weekly, providing integrated care support
- EMR: Meditech
- AI-assisted documentation technology available to streamline charting and improve efficiency
Responsibilities
- Provide comprehensive primary care services for adolescent and adult patients
- Conduct patient assessments, diagnose conditions, and develop treatment plans
- Manage acute and chronic medical conditions
- Promote preventive health and wellness initiatives
- Collaborate with physicians, nursing staff, behavioral health providers, and other healthcare professionals
- Maintain accurate and timely documentation within the electronic medical record system
Qualifications Required
- Certified Nurse Practitioner with current licensure or eligibility for licensure in New Hampshire
- Family Nurse Practitioner (FNP), or Adult-Gerontology Nurse Practitioner (AGNP) Certified
- Minimum of 2 years of Nurse Practitioner experience in a primary care setting
