A rural hospital in Northern Vermont is adding an Internal Medicine physician to a primary care practice. You will be joining 5 Providers consisting of 3 Nurse Practitioners and 2 Physicians. The support team includes a Care Coordinator, Ambulatory Pharmacist, RNs, LPNs, CMAs, LNAs, and Medical Secretaries. 

Practice Details

• Outpatient primary care practice affiliated with a rural critical access non-profit hospital.
• A supportive team of experienced providers and staff in a collaborative environment
• Provide care to patients aged 19 years and older.
• Meditech EMR
• Recognized NCQA patient-centered medical home
