• Join a well-established, 100% physician owned team of 16 providers who focus on value based care
• Mostly Outpatient with some inpatient rounding on the weekend a provider is on call
• Full and part-time positions available 
• 3 Clinic Locations - 2 in Joliet and 1 in Wilmington
• Affiliated with 2 Hospitals located just 13 miles apart
• Nursing Home coverage is available, but not required 
• Shared call of 1:6 weekends
• Wired with electronic medical records
• Patient Centered Medical Home Level 3
• In house diagnostics to make diagnosis at point of care
• Earning potential over $300,000
